The NMC SVT1 accident prevention tag is made of 15 mil vinyl for water and chemical resistance, is red with black text for high visibility when hung on a scaffold, and the front reads, "Danger - Do Not Use This Scaffold - Keep Off" with areas at bottom labeled "Date," "Competent Person," "Signature," and "Comments." Further text on the front of the tag reads, "This scaffold is being erected, taken down or has been found defective. Do Not Alter" to help describe the purpose of the tag. This tag can be used to help keep workers off of an unsafe scaffold. The back of the tag reads, "Daily Inspection Record," followed by a blank grid for entering the date and a signature for inspections. It includes a 3/8" diameter hole for fastening and a reinforced brass grommet for longer wear. The tag measures 6 x 3 inches (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
